Design guidance
- SHOULD record timeline, impact, root cause, detection, resolution and prevention for every incident.
- MUST record impact, timeline, direct cause, root cause, systemic issues, notifications and preventive measures.
- SHOULD be approved by the incident manager, system owner and compliance officer.
- SHOULD convert lessons learned into preventive measures tracked to completion and shared across the organization.

- Example: vector-DB unresponsiveness raised latency 2 s -> 15 s for 33 min, 8,450 requests, 2.5% error rate vs 0.1% normal, ~$150 excess cost; fixed with 5 s timeout, retry and circuit breaker (Ref8.06).
- Post-incident review held within 1 week of resolution (Ref9.08).
- Regulatory notification usually within 72 hours (GDPR) or 30 days (other regimes) (Ref9.08).
